Knowing what each week holds is what lets you plan time off, travel and later operations with confidence. Below, every MTF procedure's recovery sits side by side, using the same figures as the recovery timeline on each procedure page. For the detail of one operation, go to its own page. Your surgeon's discharge sheet overrides all of it.

"Desk work" means sitting at a screen, ideally from home first. Anything involving lifting, standing all day or using your voice for a living comes later, and time off work breaks it down by job.

A facial feminization bundle under one anaesthetic mostly follows its slowest piece, usually the forehead or jaw, rather than adding up, though the first week is harder and the swelling bigger.

Days 0 to 3

After breast, facial and voice work most people go home the same day or after one night, with someone to collect them and stay the first night or two. After vaginoplasty you are in hospital, mostly flat in bed with a catheter and packing in place, and moving very little. That is deliberate, because the graft needs stillness to take.

What is normal at this stage is soreness, tightness, grogginess and nausea from the anaesthetic that clears by day two. Swelling builds rather than settles, peaking around day three after most operations. After facial work the face can look startlingly puffy and bruised. After vaginoplasty the swelling is heavy and tells you nothing about the final result.

What is not normal is one side swelling far more than the other and getting harder and more painful, especially after breast or facial surgery. That can be a haematoma, and it is the one early problem where hours matter. After any neck or voice operation, noisy breathing or shortness of breath at rest is an emergency.

Week 1

After vaginoplasty this is the week the packing and catheter come out, usually between days four and seven, and the first dilation happens with a nurse in the room. Many people are discharged to a nearby hotel around now. Peeing feels strange and sprays at first. That usually settles as the swelling goes down.

After breast augmentation the first check is at about a week. After facial work, stitches and any nasal splint usually come out at five to seven days. After voice surgery the week is silence, which is harder than the pain.

Across every operation, a fever, spreading redness or a wound that starts to smell is a reason to ring rather than wait. A calf that swells and aches on one side, or sudden breathlessness, is different. It can mean a clot, which needs emergency care rather than a callback.

Weeks 2 to 4

For breast, facial and voice work, this is when most people feel roughly themselves again, if tired by mid-afternoon. Bruising fades, desk work becomes realistic, and pain drops to something paracetamol handles. Voice patients start speaking again in rationed minutes.

After vaginoplasty these are the dilation-heavy weeks: several sessions a day, sitting still uncomfortable, and discharge and swelling ongoing. Most people travelling for surgery fly home somewhere in weeks three to four, once the surgeon has seen them dilating confidently.

Weeks 6 to 8

This is when restrictions start to lift, gradually and on your surgeon's word rather than the calendar's. Breast and facial patients are usually cleared for most exercise. After vaginoplasty, desk work becomes realistic, the dilation schedule eases a little, and granulation tissue is often treated in clinic around now. It is common and simple to deal with.

Months 3 to 6

After vaginoplasty most of the swelling has gone, light exercise is back, and penetrative sex is usually allowed from around month three. Breast implants drop and soften into their final shape, and fat grafts show how much volume has stayed. Facial swelling has mostly gone, though the last of it, especially around the nose and jaw, takes longer. Voices fill out and therapy shifts from protecting the voice to shaping it.

A low patch in these months is common once the adrenaline has gone and life is ordinary again. Expect it, know that it passes for most people, and mention it to your surgeon or GP if it lingers.

Six months onward

This is the window when results are judged and any revision is planned. Surgeons want swelling fully gone before deciding what needs work, so a labiaplasty after vaginoplasty, a touch-up after facial surgery or a second voice procedure is usually discussed at around a year. People who push for a revision at month three often find the thing that bothered them has settled.

After vaginoplasty, dilation by now has dropped to a maintenance rhythm. It never quite stops, and the people who keep it steady through this calmer stretch are the ones who keep their depth.

Planning travel around it

If you are travelling for surgery, the timeline sets how long you stay. For vaginoplasty, plan on three to four weeks near the surgeon: the hospital stay, the first weeks of dilation under supervision, and a check before you fly. For facial feminization, plan on two to three weeks, which covers stitches out and the first review. For breast augmentation, about two weeks. The international patients I have seen get caught out had booked their flight home before they knew how their first week would go. Travelling for surgery has the rest.
