MTF Surgery Costs
The same operation can be free through a public health system, cost a deductible through a US plan, or run to tens of thousands paid privately. Which of those applies to you comes down to three things: where you live, who is paying and how long you can wait. Start with the numbers, then the systems that decide what you actually pay.
The numbers
Costs by procedure has self-pay ranges for every MTF procedure in the US and Thailand. They match the tables on each procedure page. To give a sense of scale, US vaginoplasty is typically $20,000 to $35,000 and breast implants $6,000 to $12,000. A full facial feminization plan can run well into five figures.
Thai hospital packages for the same operations usually come to around half the US figure, often less. That is before flights and the weeks in Bangkok. It is a large part of why Thailand has led the world in vaginoplasty and FFS for decades.
Budget beyond the surgeon's quote, too. Hair removal before vaginoplasty, dilators, travel, a companion's time and time off work all sit outside it.
Who pays
In the US, insurance is where most of the complexity lives. Plan wording, in-network surgeons, medical necessity and appeals matter more than any sticker price. Vaginoplasty and orchiectomy are the procedures most often approved. Facial and voice work is more often refused as cosmetic, though coverage is widening plan by plan.
In the UK, the NHS pathway funds genital surgery after a gender clinic assessment, but the waits run to years. Waiting years for an operation you already know you need is hard, and it is why so many UK readers choose to pay privately, at home or abroad.
Paying yourself
Surgery abroad works through the arithmetic for Thailand, the destination most trans women choose when they self-fund. It shows what you save once flights and a longer stay are added, and how to vet a team from a distance. I have coordinated patients through Bangkok hospitals for more than ten years, so this is the route I know inside out.
If the total is more than you have saved, financing prices out the borrowing, with a worked deferred-interest example. It also covers grants and crowdfunding.
Whatever route you take, ask for the quote in writing. It should name anaesthesia, hospital nights, follow-up and the revision policy line by line. An "all-inclusive" price that does not list them usually is not.
Every figure here is a planning range, dated by each page's review line. Your written quote from the surgeon or hospital is the number to plan around.
