Costs by Procedure
On this page
Vaginoplasty in the US typically runs $20,000 to $35,000 self-pay, and the same operation in a Bangkok hospital package often costs around half that. Below are the self-pay ranges for every MTF procedure side by side, matching the Costs table on each procedure's own page. Everything is in US dollars unless marked in pounds, rounded, and dated by the review line at the foot of the page.
These are the figures surgeons and hospitals quote at consultation. They are not what an insurer pays, and they are not a quote to you. If you have coverage, your out-of-pocket cost is a different question, covered under insurance and the NHS.
Thai hospitals price in baht, so the dollar figures move with the exchange rate. A Thai figure is usually a hospital package with the nights in hospital included. It leaves out the flights and the weeks you spend in Bangkok afterwards. Surgery abroad does that arithmetic. I have spent more than ten years coordinating patients through Bangkok hospitals, so these are the numbers I see on quotes every week.
Bottom surgery
| Procedure | US self-pay | Thailand self-pay | Notes |
|---|---|---|---|
| Penile inversion vaginoplasty | $20,000–$35,000 | $12,000–$22,000 | Most US quotes land mid-band; hair removal and time off sit outside it |
| Peritoneal pull-through | $25,000–$40,000 | $16,000–$28,000 | Robotic time and a second surgical team drive the spread |
| Intestinal vaginoplasty | $30,000–$50,000 | $18,000–$30,000 | The longer hospital stay is much of the difference |
| Vulvoplasty | $15,000–$28,000 | $9,000–$17,000 | Roughly a fifth to a quarter less than full depth from the same surgeon |
| Orchiectomy | $3,000–$8,000 | $1,500–$3,500 | Day case; outpatient centres at the low end |
| Labiaplasty and revision | $5,000–$12,000 | $2,500–$6,000 | Often free inside the original surgeon's revision window |
Vaginoplasty is the largest single bill most trans women face. It is also the one where the all-in quote matters most, because it carries more hospital nights than anything else here.
Breast
| Procedure | US self-pay | Thailand self-pay | Notes |
|---|---|---|---|
| Implants | $6,000–$12,000 | $3,500–$6,500 | Most US quotes $7,000–$10,000 |
| Fat grafting | $5,000–$11,000 | $3,000–$6,000 | Donor liposuction often priced per area on top |
| Implants with a lift | $9,000–$18,000 | $4,500–$9,000 | A staged plan costs more, as theatre is paid twice |
| Revision and exchange | $4,000–$14,000 | $3,000–$8,000 | A straight exchange at the bottom, capsulectomy at the top |
Facial feminization
| Procedure | US self-pay | Thailand self-pay | Notes |
|---|---|---|---|
| Forehead and brow | $12,000–$30,000 | $6,000–$14,000 | Setback costs more than shaving |
| Hairline advancement | $6,000–$15,000 | $3,000–$8,000 | Much less as an add-on to forehead work |
| Eyelid surgery | $4,000–$9,000 | $1,500–$4,000 | Adds far less inside an FFS bundle |
| Rhinoplasty | $8,000–$16,000 | $3,500–$8,000 | Rib cartilage pushes it higher |
| Cheek augmentation | $3,000–$10,000 | $1,500–$5,000 | Fat at the lower end, implants at the upper |
| Cheekbone reduction | $7,000–$15,000 | $3,000–$7,000 | Usually quoted inside a larger bundle |
| Lip lift | $3,000–$7,000 | $1,200–$3,000 | Often about an hour of theatre time |
| Buccal fat removal | $2,500–$6,000 | $800–$2,500 | Almost always bundled |
| Jaw and chin | $8,000–$25,000 | $4,000–$11,000 | Jaw and chin priced as separate items |
| Tracheal shave | $3,000–$7,000 | $1,500–$3,500 | A modest add-on on the same day as other facial work |
| Facelift and neck lift | $12,000–$30,000 | $4,000–$9,000 | Less when added to an FFS bundle |
FFS is almost never bought one row at a time. Most people book two to five procedures under one anaesthetic. A combined quote is normally well under the sum of the rows, because you pay for one theatre booking, one anaesthetic and one hospital stay.
Body and voice
| Procedure | US self-pay | Thailand self-pay | Notes |
|---|---|---|---|
| Liposuction and fat transfer | $6,000–$15,000 | $3,500–$8,000 | Roughly $3,000–$5,000 per area harvested in the US |
| Hip and buttock implants | $9,000–$18,000 | $5,000–$10,000 | Both areas together at the top |
| Wendler glottoplasty | $8,000–$15,000 | $3,000–$6,000 | Ask whether voice therapy is included |
| Cricothyroid approximation | $9,000–$17,000 | $4,000–$8,000 | A specialist operation, so quotes vary from surgeon to surgeon |
The UK
UK private prices, where providers publish them, run roughly as follows: vaginoplasty £18,000 to £28,000, orchiectomy £2,500 to £5,000, breast implants £5,000 to £8,000 and Wendler glottoplasty £6,000 to £10,000. The private market for genital surgery is small. Many UK readers either wait for the NHS pathway or travel, and the UK figures on each procedure page show which.
Why the ranges are wide
What the quote covers. Some quotes are all-in: surgeon, anaesthetist, hospital nights, medication, garments and follow-up. Others are the surgeon's fee alone, with the rest arriving as separate bills. That is the most common reason a figure moves after someone thought it was settled. I ask for anaesthesia, hospital nights and follow-up to be named line by line on every quote I handle.
Facility fees. In the US the surgeon's fee can be a third or less of the bill. The same operation in a hospital rather than a surgical centre can be thousands apart.
Bundling. FFS and breast work are often combined, and vaginoplasty sometimes includes a later labiaplasty. A bundle changes the maths in your favour, so ask for it priced both ways.
Geography and demand. Big US cities cost more than smaller ones. The most sought-after surgeons price at the top of the band and carry the longest waits.
Using these numbers
Use them to sanity-check a quote, not to shop on price alone. A vaginoplasty quote far under the bottom of a band is a reason to ask what is missing. In all my years in Bangkok, the things that predicted a great result were always the surgeon's volume, their healed results on bodies like yours, and how they look after patients when something needs attention.
The surgical fee is not the whole budget either. Hair removal before vaginoplasty is paid session by session over a year or more; see hair removal. Add dilators and supplies, time off, and travel if you are going abroad. If the total is more than you have, financing sets out the options. Every figure here is a planning range. Your written quote from the surgeon or hospital is the number to plan around.
